Diabetes
Education Program
Our Diabetes Education Program is ADA and State of Iowa
certified. We offer education to patients and outpatients
about diabetes management. Individuals can schedule office
visits with the Diabetes Nurse Educator and/or Registered
Dietitian/Educator for review of diet or self-management
concerns. The two also coordinate the monthly meeting of
the Diabetes Support Group.
Services Offered:
- Diabetes Comprehensive Self-Management Course – Recognized
by the American Diabetes Association for Quality Patient
Education. Referring physician’s goals/treatment
plan is emphasized. Complete review of diabetes covers
the following: physiology of diabetes, acute and chronic
complications, medication management, monitoring, insulin
injection technique, nutrition basics, meal planning, special
nutritional considerations, exercise, proper foot care,
emotional aspects of diabetes and current developments
in diabetes management. Patient instructed on individualized
meal plan by a registered, licensed dietitian. Patient
develops one goal to improve glucose control. Additional
counseling/support provided post-program, as needed. Pre-and
post-program tests assess patient’s response to instruction.
Instruction provided and patient progress/follow-up reports
sent to the referring physician. Family member or other
caregiver instructed at same time at no additional cost.
Program length: 10 hours total.
- Insulin Administration Instruction- For insulin-requiring
patients. Demonstrate proper technique for drawing single
or mixed doses of insulin as prescribed by physician. Review
treatment goals, insulin action times and proper storage,
causes/symptoms/treatment of hypoglycemia and hyperglycemia.
Show appropriate injection sites. Patient gives return
demonstration of proper insulin drawing and injection technique
by self-injecting normal saline. Family member or other
caregiver instructed at same time at no additional cost.
Report sent to referring physician. Approximate length
of visit: 1 hour.
- Gestational Diabetes Class- Review etiology of gestational
diabetes, desired blood glucose levels, effects of hyperglycemia
on fetus and treatment options. Patient receives a full
nutritional assessment by a registered, licensed dietician
and includes a thorough review of the role of diet in the
control of blood sugar. Patient receives an individual
meal plan tailored to the nutritional needs of pregnancy.
Patient also receives blood glucose monitor and monitoring
instruction. Patient progress reported to referring physician.
Family member or other caregiver instructed at same time
at no additional cost. Approximate length of program: 2
hours.
- Office visits with Dietitian and/or nurse Education -
for review of diet or self-management concerns.
- Diabetes Support Group - meetings are provided as a public
service to diabetic patients and their support person.
Held
on the second Wednesday night of the month at 7 p.m. in
the RSVP building at 749 Second St. in Webster City. There
is
no meeting in December. The meeting agenda includes a short
educational program, time for discussion and questions,
and social time with refreshments provided.
