Patient Accounts

Our Patient Accounts Department is here to help you with the paper work of healthcare. Pre-authorization, billing, liability claims, Medicare and Medicaid are just a few of the areas in which we can provide assistance.

Pre-authorization
Your insurance plan may require notification for some hospital services. Please check your card, policy or carrier for their requirements. If you need any assistance with this, Hamilton Hospital staff will be glad to assist you.

Billing
Hamilton Hospital does the billing for the hospital charges plus billing for Anesthesiologist, Emergency Room Physicians, Home Care, Jewell Clinic, Stratford Clinic and Dr Andrew.

Homecare statements will be separate from the hospital.

You may also receive bills from other providers for related services. Such as Radiologist, Pathologists and Physicians.

Physical therapy, occupational therapy and cardiac rehabilitation will be billed to your insurance company at the end of each month.

Please present your insurance cards each time you visit to ensure prompt and accurate filing of you insurance.

Your insurance company may deny or hold claims pending information for such reasons as; possible third party liability, pre-existing condition, possible work-related, or coordination of benefits. Please respond to all requests for information from your insurance company.

Liability Claims- Can be an accident or injury on someone else's property or in a motor vehicle. The bill will be mailed to the patient for them to forward to the responsible party. We will submit to the Liability Insurance when supplied with that information. We can file with your health insurance in cases where it is going through litigation. Your health insurance company will probably send out a questionnaire to obtain information on the accident. You must respond before they will issue payment.

Worker's Compensation - The employee should fill out an injury report from their place of employment. You may be required to see the company doctor. The bill will be sent to the employer for them to forward to their insurance carrier. On Inpatient admissions the employer will be contacted to inform their insurance carrier for purposes of utilization review

Medicare
Hamilton Hospital accepts assignment on Medicare's allowable charges.

Medicaid
Please present your Medicaid card on each visit. You should contact your physician before presenting to the hospital except in the case of a life threatening illness or injury.

You may be financially responsible if your visit is found to be not medically necessary.

Medicaid is always the payer of last resort.

General Medicaid - You will have the choice of your physician and provider of services.
Medipass - You are assigned to a primary physician. Prior to treatment by another physician you have approval from your primary care physician, except in the case of a life treating injury or illness.

Charity Care Program
Hamilton Hospital believes that all people should get medical care whether or not they can pay. Eligibility for the program will be determined by the family's income and number of family members. Uninsured persons who are up to 125% of the poverty level may qualify for 100% discount, and persons with income up to 200% of the poverty level may qualify for a discount on a sliding scale.

Persons unable to pay for their medical expenses may qualify for Hamilton Hospital's Charity Care Program. For more information, stop in the hospital's Business Office, or call 515-832-7715.

Payment Guidelines
Hamilton Hospital does not charge interest on accounts, but we must have substantial monthly payments on account to meet our financial obligations.

These payment guidelines are for the patient responsibility portion of your account.

Please contact our office to confirm your payment arrangements.

You can pay your hospital bill by credit card, in person, through the mail or by phone. We accept MasterCard or VISA.

A hospital does not serve in order to collect money, but it must collect in order to serve.

If payments are not made regularly accounts will be referred to a collection agency.
Currently Hamilton Hospital uses H & R Accounts out of Cedar Rapids to collect our delinquent accounts.
